Certificate of Immunization, Form DH 680, is the only document that the
schools are permitted to accept as proof of immunization. |
| Shots
may be obtained through your local physician or Palm Beach County Health
Department Health Centers or Palm Beach Health Department
Immunization Van
located throughout the county. |

Diphtheria/Tetanus/Pertussis (DTaP)
Polio Series
Measles/Mumps/Rubella (MMR)
Hepatitis B Series (Hep B)
Varicella
Pneumococcal –children 2-24 months entering childcare
Tetanus/Pertussis Booster (Tdap) 7th grade
Haemophilus influenzae type B (age appropriate)

By the time a child starts school he/she should have already had a number of
required immunizations. However, proof of updates, boosters, and series
completion is required to enter Palm Beach County Schools for the first time
and to advance to some grade levels. A physical is required for entrance to
Pre-kindergarten, kindergarten and those entering 7th grade. |
