    PALM BEACH COUNTY, FL The Palm Beach County Health Department encourages parents of school age children to vaccinate early well before the back to school rush. Childhood immunizations are available by appointment at our Health Centers and the following outreach locations with our Immunization Van.

        New students in Palm Beach County will need a "680" blue card from Florida, with the vaccines received in the other location.
Kindergarten students( age 4-5) - Boosters for DTaP, Polio, MMR and Varicella( chicken pox)( In addition to all other vaccines)
1st and 2nd grade - 2 Varicella ( in addition to all other vaccines)
7th & 8th Grades( age 11-12) - Tdap booster ( in addition to all other vaccines)
8th and 9th grade - At least 1 varicella is required.( in addition to all other vaccines)

Physicals are also required for new students, and students in Kindergarten and 7th grade.

Please bring your child's shot records for review. Vaccine ( shot )records are required if we are to safely immunize your child!!!
