Birth & Death Certificates

Birth Certificates:

 


Birth certificates are available for persons born in the state of Florida for the years 1919 through the present.

If you were born before 1930 you must apply through the State Office of

Vital Statistics at
P.O. Box 210, Jacksonville, FL 32231
.

    Birth certificates are confidential and can only issued to the registrant, if of legal age (18), parent, guardian or lega1 representative or upon court order. Birth certificates become public record after 100 years following the birth. 

The fee for one certified birth certificate is $14.00
 
Instructions:
To order a birth certificate, download the application form and complete all information:

  • Information about the person making the request:
  • Full name
  • Relationship to the person named on the record
  • Mailing address
  • Photocopy of ID
  • Telephone number where you can be reached during our office hours

Deaths Certificates:

 


The death you are concerned with must have occurred in Palm Beach County. We only have records available for the years 2007 through the present. If the person died in the state of Florida prior to 2007 contact

For deaths outside of Florida contact the state where the death occurred. 

If you are requesting the cause of death, you must explain your relationship to the person whose record you are requesting. If a relationship is not stated, you will be issued a certified copy excluding cause of death information.

By law, the death records, which include cause of death information, can only be issued to:

  • The decedent's immediate family or guardian (spouse, parent, and if of legal age child, grandchild, or sibling, next of kin or guardian)
     
  • To a representative (attorney, executor, physician, funeral director, insurance agent or anyone with power of attorney) as specifically authorized by the immediate family.
     
  • A stepchild, grandchild, grandparent or other relative of the deceased will qualify if such person provides documented proof of a direct financial interest in the cause of death
     
  • A court order requires issuance of cause of death. Cause of death information becomes public information 50 years following the death.


The fee for one death certificate is $10.00

To order a death certificate, download the application form and complete all information:

  • Information about the person whose record is being requested:
    • Full name of the deceased person
    • Sex
    • Date of death
    • City of death
  • Information about the person making the request:
    • Full name
    • Relationship to the decedent, if requesting cause of death information, Photocopy of ID is required
